Before your appointment

After scheduling your pet’s spay/neuter appointment, an electronic surgery consent form will be emailed to you.  It is important that this form is filled out and submitted before the day of your pet’s appointment to avoid delays at surgery check-in.

If you did not receive this form, please call us at (214) 742-7722 or email us at spca@spca.org so we can confirm your email and resend.

If your pet is current on vaccinations, please attach the proof of these vaccinations to the consent form or send them via email to clinic@spca.org.
